Regional Credit Center?

I just received a copy of my TU credit report, and there is a rather mysterious listing on it:
Creditor: Regional Credit Center
Address: PO Box 60511
Phoenix, AZ 85082
There is no phone number available for them. It is listed as a revolving charge account opened 10/94, paid off 12/97, but with a bunch of 90-day lates. I have no idea what this is. It couldn't be mine because I was in Basic Training for the Army in 10/94. Anyone know who these people are? Any ideas on how to get rid of this? I can't send a validation letter CRRR, I usually fax them, but there is no phone number to get a fax #. UGH!!!! Suggestions welcome! Thanks all.
