Get a lawyer- Check her credit reports, looks to me like ID theft. If so, file a claim with NY Attorney General's office, FTC and your County sheriff. If the card was never activated by your mother, then SOMEONE else probably requested a duplicate card to another address, actvated it and ran it up.
